In brief

To start a divorce, legal separation or nullity case in Riverside County the court’s minimum set is the summons (FL-110), the petition (FL-100), the local Declaration of Residence (RI-FL036) and, if there are children under 18, the UCCJEA declaration (FL-105). Later forms follow the stage of the case: proof of service, financial disclosures, a response, and then the judgment papers for the path the case takes.

  • RI-FL036 is a Riverside local form; the court’s FAQ lists it with FL-110 and FL-100 as part of the minimum set1.
  • Financial disclosures (FL-140, FL-142 or FL-160, FL-150) are required within 60 days of filing; the only disclosure form filed with the court is FL-14113.
  • A respondent answers on form FL-120 within 30 days of service13.
  • A default judgment needs FL-165, FL-170, FL-180 and FL-190, plus child-related attachments when there are children13.
  • Riverside’s Self Help Legal Services groups the required forms into digital packets with an online cover sheet4.

This page explains the court’s process. It is not legal advice, and this site is not a law firm.

The detail

This page lists the forms in the order a case uses them, with the form number, its name, when it is used and a link to the Judicial Council’s PDF. For the steps around them (where to file, serving, the six-month wait), see how to file for divorce in Riverside County. Fees for the filings are on Riverside County divorce filing fees.

The short answer

If you are… The forms are
Starting a divorce, legal separation or nullity case FL-110, FL-100, RI-FL036, and FL-105 if there are children1
Serving the papers FL-115 (proof of service), or FL-117 if the other spouse signs for them3
Sharing finances FL-140 with FL-142 or FL-160, and FL-150; FL-141 is filed13
Responding to a petition FL-120, and FL-105 if there are children13
Finishing a case with no response FL-165, FL-170, FL-180, FL-1901
Asking the court to waive fees FW-001, with the top of FW-0035

Revision dates are the ones printed on the copies of the forms the sources cite. The Judicial Council revises forms from time to time, so the date on the form in hand is the one that counts. Each form number below links to the PDF at the address the Judicial Council prints for it3.

Step 1: Start the case

The court’s FAQ gives the minimum paperwork for a divorce, legal separation or nullity, and says “the process is essentially the same” for all three1.

Form Name When it is used
FL-110 Summons (Family Law) Served with the petition; gives 30 days to respond; page 2 carries the standard restraining orders (revised January 1, 2015)225
FL-100 Petition: Marriage/Domestic Partnership Starts the case (revised January 1, 2020)6
RI-FL036 Declaration of Residence (Riverside local form) Marked mandatory in the court’s packet; says which court should hear the case12
FL-105 Declaration Under UCCJEA (GC-120) Required when there are children under 1813
RI-FL011 Confidential Contact Information Optional; to receive the court’s self-help emails by email2
FL-311 Child Custody and Visitation (Parenting Time) Application Attachment Optional, attached to the petition or response3
FL-160 Property Declaration Optional at this stage; also an alternative to FL-1423

Sources disagree The copy of RI-FL036 in the court’s older dissolution packet (revised January 1, 2013) offers Blythe, Hemet, Indio and Riverside as the court that should hear the case2. The court’s Family Law page now lists four family law courthouses: Riverside, Blythe, Indio and the Menifee Justice Center7. The 2026 list is the current one; ask the clerk which choices the current form offers. Which courthouse takes which address is on which courthouse takes your family law case.

The Judicial Council’s guide also points to the blank response (FL-120) and the proof of service (FL-115) at this stage, because the blank response goes to the other spouse with the summons39. Copies: the Judicial Council’s steps call for two copies of the petition and summons, and the court’s packet asks for the original and at least one copy (maximum two)28.

Riverside’s Self Help Legal Services has grouped the required forms into a digital packet. A person completes an online cover sheet that fills contact information into the forms, and the court notes that “filling in the coversheet only partially completes your forms,” so the packet is saved locally first4.

Step 2: Serve the papers

Form Name When it is used
FL-115 Proof of Service of Summons The server completes and signs it; the petitioner files the original39
FL-117 Notice and Acknowledgment of Receipt Service by mail: service is complete the day the other spouse signs10

The Riverside Notice of Case Assignment and the Family Law Case Management Order are served with the petition under Local Rule 514511. The service methods are on serving divorce papers in Riverside County.

Step 3: Share financial information

Preliminary disclosure is due within 60 days of filing the petition, or of filing the response for the respondent (Family Code section 2104(f))12. The disclosure documents are exchanged, not filed, and the parties file proof that they were served12.

Form Name When it is used
FL-140 Declaration of Disclosure Cover sheet for preliminary and final disclosure; not filed (revised July 1, 2013)
FL-142 Schedule of Assets and Debts Part of disclosure, or use FL-160
FL-150 Income and Expense Declaration Part of disclosure; also used with support requests
FL-141 Declaration Regarding Service of Declaration of Disclosure Filed to show disclosure was served or waived (revised July 1, 2013)
FL-144 Stipulation and Waiver of Final Declaration of Disclosure Both spouses waive the final disclosure (revised January 1, 2007)

Source: the Judicial Council’s forms list and the form PDFs3. The court’s flow chart (revised November 10, 2020) has FL-142, FL-150 and FL-141 filed right after the petition13. The disclosure rules are on the divorce disclosure forms.

Step 4: Respond to a petition

Form Name When it is used
FL-120 Response: Marriage/Domestic Partnership The respondent’s answer, due 30 days after service13
FL-105 Declaration Under UCCJEA With the response when there are children3
FL-160, FL-311 Property Declaration; custody application attachment Optional3
FL-335 or FL-330 Proof of Service by Mail; Proof of Personal Service The proof for the response3

The response process is on served with divorce papers in Riverside County.

Step 5: Ask for orders during the case

Form Name When it is used
FL-300 Request for Order Custody, support or any other order; revised July 1, 2025
FL-320 Responsive Declaration to Request for Order The other side’s reply (revised July 1, 2025)
FL-340 Findings and Order After Hearing After a hearing

Source: the Judicial Council’s forms list, the form PDFs and the Riverside packet314.

Riverside’s Request for Order packet lists FL-300 as the required form, with FL-150, FL-330, FL-335 and the interpreter request RI-IN00714. The fee for a request is on the filing fees page.

Step 6: The judgment papers

The forms depend on the path to judgment. Riverside has fillable packets for a default judgment, a default judgment by agreement, and a judgment by agreement15. Details are on finishing a divorce in Riverside County.

Form Name Used for
FL-165 Request to Enter Default After 30 days with no response (revised July 1, 2025)
FL-170 Declaration for Default or Uncontested Dissolution or Legal Separation Default and uncontested judgments (revised January 17, 2020); item 12 asks to restore a former name
FL-130 Appearance, Stipulations, and Waivers Uncontested judgment by agreement; both sign
FL-180 Judgment The final order (revised July 1, 2012); item 4(f) for a former name
FL-190 Notice of Entry of Judgment Mailed by the court after the judgment is signed
FL-182 Judgment Checklist Lists what the court may require; optional to file
FL-157, FL-343, FL-435 Spousal support declaration, order attachment, earnings assignment When spousal support is part of the judgment
FL-345 Property Order Attachment to Judgment Property orders

With children the default path adds FL-341 (custody and visitation order), FL-342 (child support information and order), FL-195 with FL-192 (income withholding and rights notice) and FL-191, the Child Support Case Registry Form, which is not attached to the judgment1617. Sources for the table are the Judicial Council’s default, default-with-children and written-agreement pages and the forms3161718.

A contested case uses the local request for a settlement conference and trial, RI-FL008, under Local Rule 515011.

When to get help

The court’s Self Help Legal Services offers free self-help resources and can explain court procedures, but staff cannot tell anyone which choice to make24. The Self-Help Center describes the locations and workshops. A reason to get help before filing is a case with children or property in dispute, where the choice of forms follows from the facts. A legal aid organization or an attorney can review the packet; free and low-cost help in Riverside County lists the free routes first.
